As little as a 5%-10% reduction in weight can diminish the risk for diseases such as coronary heart disease and type 2 diabetes.

Danielle Ostendorf, MS, PhD, CEP, an Assistant Professor at the University of Colorado Anschutz Medical Campus, specializes in promoting long-term physical activity adherence in adults with overweight or obesity. Her work emphasizes the importance of understanding the psychological and physiological factors that influence weight loss.
Counteracting Physiological Mechanisms
A key finding that Ostendorf highlights is that when someone attempts to lose weight, there are physiological mechanisms at play that try to counteract the weight loss attempts. These include feeling less full after eating, increased hunger, and burning fewer calories at rest. High levels of physical activity can help to counteract these physiological mechanisms and promote weight loss maintenance.
Intrinsic Motivation and Enjoyable Activities
Another key finding is that when people find physical activities that they enjoy, they are much more likely to keep up with physical activity long-term. Ostendorf's research confirms that people who are motivated to engage in exercise for more intrinsic reasons (enjoyment) are much more likely to maintain their increases in physical activity 12 months after the program started.

The Importance of Messaging
Ostendorf believes that our messaging that promotes behavior change is very important. We have to create messages that are personally motivating to people and ideally focus on the more short-term benefits. Humans tend to favor immediate, short-term rewards (such as less stress, more energy, and better sleep) over long-term benefits (such as disease prevention). So when trying to motivate a person to engage in a healthy lifestyle, it may be more effective to have them focus on things that are happening that day or that week, instead of a year from now. For example, the idea of less stress may be more motivating in the moment compared to the idea of preventing chronic diseases, which can happen, but much later down the road. Exploring Innovative Weight Loss Programs
Virtual World Weight Loss Programs
Traditionally, weight loss programs have been delivered via face-to-face programming either in the community or in a commercial or clinical setting. Over the last decade or so, Internet or Web-based programming has emerged. However, results of Web-based weight loss interventions have been highly variable and often with small effect sizes.
In contrast to Web technologies, 3-dimensional (3D) virtual reality technologies more directly support engagement and active learning. Virtual worlds share many of the strengths of virtual reality technologies, particularly the rendering of 3D spaces. However, they are more accessible to users (via an Internet-connected personal computer) and thus may offer a way to extend the reach of programs to obese and overweight individuals.
Club One Island: A Case Study
Club One Island is an interactive weight loss community delivered via Linden Lab’s Second Life, an online virtual world. Club One Island provides participants with a professional team, education, and specialized tools to help them overcome individual barriers to weight loss. The island offers highly interactive 3D spaces (eg, a restaurant, a Mini-Mart convenience store, and an encouragement room), creative educational tools (eg, a nutritional jeopardy game and a fire pit illustrating how the body uses food as fuel), over 30 movement activities (eg, bikes and bike paths, surfing, exercise balls, lap swim, basketball, weight lifting, yoga, dancing, and rock climbing), and numerous healthy habits tools (eg, tracking charts). All elements are intended to engage participants in social networking, play, and learning. The 12-week program was delivered to cohorts of 15-20 participants for a total of 48 instructor hours. Each week, four 1-hour classes (Nutrition, Movement, Healthy Habits, and Support Group) were led by certified fitness, nutrition, and support professionals.
Study Results
A study comparing a virtual-world weight loss program to a face-to-face program showed that both groups lost a significant amount of weight (virtual world: 3.9 kg, P < .001; face-to-face: 2.8 kg, P = .002); however, no significant differences between groups were detected (P = .29). Compared with baseline, the virtual-world group lost an average of 4.2%, with 33% (11/33) of the participants losing a clinically significant (≥5%) amount of baseline weight. The face-to-face group lost an average of 3.0% of their baseline weight, with 29% (6/21) losing a clinically significant amount. Overall, these results offer positive early evidence that a virtual-world-based weight loss program can be as effective as a face-to-face one relative to biometric changes.

Endoscopic Procedures for Weight Loss
Bariatric surgery is one of the most effective ways for people living with obesity to lose, and keep off, weight successfully for the long term. However, less invasive options like Endoscopic Sleeve Gastroplasty (ESG) and gastric balloons are gaining popularity.
Endoscopic Sleeve Gastroplasty (ESG)
ESG is an endoscopic procedure that configures the stomach into a smaller shape without making external incisions or removing organ tissue. A specialized sewing device mounted on an endoscope then sews the stomach from the inside to reduce its capacity by 70% to 80%.
Gastric Balloon
Another lesser known, but effective, endobariatric option for weight loss is a gastric balloon. During this outpatient endoscopic procedure done under anesthesia, a doctor passes a deflated balloon through the patient’s mouth, down the esophagus and into the stomach. Once confirmed to be in correct position, the balloon is filled with fluid until it is about the size of a grapefruit. Patients continue to receive medical supervision for six months or longer to ensure they’re confident in the new habits they’ll need to keep the pounds off.
Patient Experience: Ellen Frazier's Journey
As it did for many people, Ellen Frazier’s relationship with her body began to change during the COVID-19 pandemic. Over the course of two years, Frazier gained 40 pounds. She tried to lose the weight by dieting, but it always came back, and with a few extra pounds to boot. The turning point for Frazier came when she saw 260 pounds on the scale. Frazier ultimately decided to undergo ESG. After her procedure, she had to learn to adjust to a new way of eating, starting with a liquid diet and gradually re-introducing solid foods. “It requires a complete mindset shift,” she says. “Before having any type of procedure, take a deep look inward and find out what it is that makes you lean into food and overeating."

Study on ESG Effectiveness
In a randomized study, patients with class 1 or class 2 obesity (BMI 30 to 40 kg/m2) who underwent ESG in conjunction with prescribed diet and exercise counseling achieved an average of 13.6% total body weight loss (TBWL) at year one, compared to 0.8% with lifestyle modification alone. In that study, 64.7% of patients who underwent ESG in conjunction with lifestyle changes lost at least 10% TBWL compared to 4.5% of patients with lifestyle modification alone. Approximately 21.2% of patients lost more than 20% of their body weight. Optimal results were achieved when patients committed to long term lifestyle changes around diet and exercise.
Future Directions in Weight Loss and Fitness
Danielle Ostendorf is developing and researching a physical activity support program to help adults adopt and sustain the high levels of physical activity that are needed for weight loss maintenance. The goal of this program is to teach people that all movement counts and to focus on finding and doing physical activities that are enjoyable. Her hope is that this program can be offered at the AHWC and other locations for an affordable cost so we can help as many people as possible. Our next step is to translate this program into Spanish so it can help even more people.
