Armodafinil: Exploring its Role in Weight Loss and Wakefulness

Armodafinil, sold under the brand name Nuvigil, is a prescription medication primarily known for its wakefulness-promoting effects. It is approved by the FDA to treat excessive daytime sleepiness associated with conditions like narcolepsy, shift work sleep disorder (SWD), and obstructive sleep apnea (OSA). While its primary use is not for weight loss, some studies suggest a potential link between armodafinil and weight management. This article explores the uses of armodafinil, its potential effects on weight loss, and important considerations for its use.

Understanding Armodafinil

Armodafinil is a newer drug than modafinil, approved in 2007 as the R-enantiomer of modafinil. Enantiomers are molecules that are mirror images of each other. Armodafinil may have a longer half-life than modafinil and, in some cases, may be considered a stronger drug with better wakefulness effects.

Armodafinil and modafinil are believed to work in the central nervous system (CNS) and boost dopamine activity in the brain. Their effects may be similar to other stimulants, such as amphetamine (Adderall) and methylphenidate (Concerta). Both armodafinil and modafinil are Schedule IV drugs that carry a risk of abuse and dependence.

Approved Uses of Armodafinil

Armodafinil and modafinil are prescription drugs that are FDA-approved to treat daytime sleepiness related to narcolepsy, shift work sleep disorder, and obstructive sleep apnea. These medical conditions are characterized by inadequate feelings of rest, which results in excessive sleepiness. If you work the night shift, you might struggle with extreme sleepiness during the day. But with proper treatment like armodafinil (Nuvigil) or modafinil (Provigil), you can stay awake and feel more alert. These medications can be prescribed if you’ve been diagnosed with shift work disorder (SWD), narcolepsy, or obstructive sleep apnea (OSA).

Off-Label Uses and Research

Armodafinil and modafinil have also been studied for off-label purposes, including:

  • Fatigue: Some studies show that modafinil can be an effective treatment option for fatigue that occurs as a result of cancer and cancer treatment. Lower doses of modafinil may be effective for treating fatigue symptoms in people with multiple sclerosis.
  • Psychiatric Disorders: Other off-label uses include treatment for psychiatric disorders like depression and attention deficit hyperactivity disorder (ADHD).

Armodafinil and Weight Loss: Exploring the Connection

While not an approved indication, some research suggests a potential link between armodafinil and weight loss.

  • Study Results: In a study, patients on armodafinil lost 2.4 kg more fat than those on placebo at 6 months (95% confidence interval, 0.9-4.0; P = 0.002). Armodafinil might be a useful adjunctive to weight loss in patients with OSA rejecting conventional treatments but this needs to be directly tested in a specifically designed, properly powered clinical trial.
  • Appetite Suppression: Nuvigil can decrease your appetite, which may lead to weight loss.

It's important to note that these findings do not establish armodafinil as a primary weight loss medication. Further research is needed to fully understand the mechanisms and efficacy of armodafinil for weight management.

Driving Task Performance

Armodafinil improved driving task performance over placebo at 3 months (12.9 cm; 95% confidence interval, 4.1-21.7; P = 0.004), but not the primary time point of 6 months (5.5 cm; 95% confidence interval, -3.3 to 14.3; P = 0.223).

Dosage and Administration

Armodafinil is usually prescribed as a 250 mg tablet taken once daily. Doses of armodafinil or modafinil may need to be adjusted to improve sleepiness during the day. While optimizing doses, persistent sleepiness can still occur.

Side Effects of Armodafinil

Nuvigil can cause certain side effects, some of which are more common than others. These side effects may be temporary, lasting a few days to weeks. However, if the side effects last longer than that, bother you, or become severe, be sure to talk with your doctor or pharmacist.

  • Common Side Effects: The most common side effects of modafinil and armodafinil include headache, nausea, dizziness, and insomnia. Both medications can also cause dry mouth, indigestion (dyspepsia), and anxiety. Side effects related to weight loss can also occur. These are just a few of the more common side effects reported by people who took Nuvigil in clinical trials: dizziness, nausea, insomnia (difficulty sleeping), diarrhea, headache.
  • Serious Side Effects: Other rare but serious side effects include allergic reactions such as rash and shortness of breath. Psychiatric side effects such as depression or psychosis are also possible. This may not be a complete list. Nuvigil may cause serious side effects, but they are rare. Serious side effects that have been reported and their symptoms include: serious skin reactions, such as toxic epidermal necrolysis, which may include symptoms such as: fever, feeling generally unwell, skin rash, blisters, heart problems, which may include symptoms such as: shortness of breath, heart palpitations, chest pain, DRESS syndrome (severe reaction affecting the skin and organs), which may include symptoms such as: skin rash, fever swollen lymph nodes, mental health problems.
  • Allergic Reactions: This medicine may cause serious allergic reactions, including anaphylaxis, angioedema, or certain skin conditions (Stevens-Johnson syndrome). These reactions can be life-threatening and require immediate medical attention. Call your doctor right away if you have a rash, itching, blistering, peeling, or loosening of the skin, fever or chills, trouble breathing or swallowing, or any swelling of your hands, face, mouth, or throat while you are using this medicine. This medicine may cause serious allergic reactions that may involve multiple organs, such as your liver or kidneys. Check with your doctor right away if you have a fever, dark urine, headache, rash, stomach pain, swollen, painful, or tender lymph glands in neck, armpit, or groin, unusual tiredness, or yellow eyes or skin. For some people, Nuvigil tablets can cause an allergic reaction.

Drug Interactions

Armodafinil and modafinil share many of the same drug interactions. These medications may affect how many drugs are metabolized or processed in the body.

  • Armodafinil and modafinil may reduce the effectiveness of steroidal contraceptives. Hormonal birth control pills, implants, patches, shots, vaginal rings, or IUDs may not work as well while you are using armodafinil. To keep from getting pregnant, use a second form of birth control together with your pills, implant, patch, shot, vaginal ring, or IUD while you are using this medicine. Continue using the second form of birth control for 1 month after your last dose. Other forms of birth control include condoms, a diaphragm, or contraceptive foam or jelly.
  • Armodafinil and modafinil may increase the clearance of cyclosporine from the body.
  • Armodafinil and modafinil may also increase the levels of certain drugs, including Prilosec (omeprazole), Valium (diazepam), and Dilantin (phenytoin).

This may not be a complete list of all possible drug interactions.

Warnings and Precautions

  • Serious Skin Reactions: Both armodafinil and modafinil can cause serious rash.
  • Psychiatric Symptoms: Armodafinil and modafinil may cause psychiatric symptoms, especially in those with a history of psychosis, depression, or mania. Armodafinil may cause some people to be agitated, irritable, or display other abnormal behaviors. It may also cause some people to have suicidal thoughts and tendencies or to become more depressed. If you notice any of these side effects, tell your doctor right away.
  • Cardiovascular Effects: Stimulant-like drugs may cause adverse effects on the heart and cardiovascular system. Those with high blood pressure may need to be monitored when first starting armodafinil or modafinil.
  • Pregnancy and Breastfeeding: The use of modafinil or armodafinil is not usually recommended during pregnancy.
  • Alcohol: It is not recommended to take armodafinil or modafinil while drinking alcohol.
  • Dependence: Some people can become mentally or physically dependent on this medicine. If you have questions, talk with your doctor.

Cost and Coverage

Armodafinil is widely available as a generic medication covered by Medicare and insurance plans. Insurance typically covers both medications, but costs can vary significantly-discount programs can reduce the price. Generic Nuvigil can cost an average of over $600. Depending on your insurance plan, modafinil may be covered. In fact, Medicare and most insurance plans cover modafinil for approved purposes. The average cost of modafinil may be over $900.

Important Considerations

  • Not a Substitute for Sleep: This medicine does not take the place of getting enough sleep. It should not be used for occasional sleepiness that has not been diagnosed as narcolepsy, sleep apnea, or a shift work sleep problem. Ask your doctor for advice about good sleep habits.
  • Regular Check-ups: It is very important that your doctor check your progress at regular visits to make sure this medicine is working properly and to check for unwanted effects.
  • Driving and Operating Machinery: Do not drive, use machines, or do anything else that could be dangerous until you know how this medicine affects you.
  • Effectiveness Over Time: Check with your doctor if you have been taking this medicine for a long time and feel that it is not working as well as before. Do not increase your dose without talking to your doctor.
  • Consult Your Doctor: Do not take other medicines unless they have been discussed with your doctor. This includes prescription or nonprescription (over-the-counter [OTC]) medicines and herbal or vitamin supplements.

Managing Side Effects

  • Headache: If you have headaches that are bothersome, talk with your doctor. They can recommend ways to ease your headache symptoms, such as an over-the-counter pain reliever like ibuprofen (Advil) or acetaminophen (Tylenol). If they don’t go away or are difficult to manage, your doctor may suggest switching to a different medication to treat your condition.
  • Mental Health Problems: Before starting treatment with Nuvigil, be sure to tell your doctor if you have a history of mental health problems. They can determine whether Nuvigil is a safe treatment option for you. If you experience mood changes or symptoms of mental health side effects while taking Nuvigil, talk with your doctor. They can determine whether it’s safe for you to continue taking Nuvigil. If your symptoms are severe, seek emergency medical care right away.
  • Withdrawal: If you have concerns about dependence on Nuvigil or feel you need to stop taking it, talk with your doctor. If you and your doctor agree to stop Nuvigil treatment, they’ll recommend a dosing schedule to gradually reduce your dose over time and help avoid withdrawal symptoms.
