The Food & Home Card offers a convenient way to save money each month. Enrollees receive a pre-loaded card with a fixed amount to spend on eligible items and expenses. This benefit is available on select plans and is specifically designed for chronically ill members with conditions such as diabetes, high blood pressure, high cholesterol, heart problems, or stroke. Other conditions may also qualify, subject to meeting the plan's coverage criteria and eligibility requirements.
Eligibility for the Food & Home Card
To determine if your Devoted Health plan includes the Food & Home Card and the amount you're eligible for, visit the member portal or review your plan documents. Qualification requirements vary depending on your plan type:
- C-SNP Enrollees: A C-SNP qualification form, completed and signed by your doctor, is required before the card can be issued. Devoted Health may contact you or your doctor to confirm your eligibility.
- Other Devoted Health Plan Enrollees: Devoted Health will review your claims and medical records to verify a qualifying health condition. You may be contacted for additional information. Alternatively, you can temporarily qualify by self-attesting to a qualifying health condition.
It's important to note that the Food & Home Card is not available on all plans, and not all members will qualify.
Earning Additional Rewards
Devoted Health also offers a reward card program. If you're enrolled in a D-SNP or C-SNP plan, completing your yearly Health Risk Assessment (HRA) can earn you a $20 Devoted Dollars reward card. This card can be used for gas and groceries. The reward card arrives pre-loaded and ready to use, typically within 90 days of completing the HRA. Remember to use the card within 12 months of issuance, as it will expire.

Using Your Food & Home Card
The Food & Home Card can be used at participating grocery stores, online with CVS, or through the OTC Health Solutions app. It's designed for ease of use and flexibility in managing your monthly allowance.
Purchasing Food
Shop for qualifying foods at participating grocery stores, including Walmart, Kroger, Dollar General, Publix, Food Lion, Safeway, The Giant Company, Giant Eagle, Albertsons, and Sedano's. When paying, swipe your Food & Home Card as CREDIT. The card cannot be used at convenience stores, restaurants (including fast food), or gas stations.
While you can buy most foods, it's recommended to focus on healthy options such as:
- Fruits and vegetables
- Fish and lean meats
- Eggs, cheese, milk, and other dairy products
- Whole grains, like whole wheat bread and brown rice
- Nuts, seeds, and spreads
- Olive oil and other plant-based oils
- Supplemental drinks like Boost® and Ensure®
The card cannot be used for items like soda, candy, alcohol, baby formula, and pet food.
Paying Utility Bills, Mortgage, or Rent
Your Food & Home Card can also be used to pay for essential housing costs. To pay utility bills, mortgage, or rent, use your 16-digit card number. The payment must be made to a utility company or landlord that accepts Visa and has a qualifying Merchant Category Code (MCC). The address on the bill must match the address on file with Devoted Health.

Eligible housing costs include:
- Electricity, water, sewer, trash
- Home heating fuel (like oil or home natural gas)
- Internet and phone
- Rent or mortgage
The card cannot be used for money orders, prepaid cell phone service, cable, satellite, streaming services, or gasoline for your car. If there's a processing fee, it will be deducted from your monthly benefit.
Alternative Payment Methods
While the Food & Home Card is the easiest way to use your funds, you can also pay out-of-pocket and request reimbursement. To do so, fill out a CVS/OTC Health Solutions reimbursement request form, available online or by calling 1-855-904-3663 (TTY 711). You'll need to provide documentation proving your purchase qualifies, including:
- For Food and OTC purchases: A copy of the receipt showing the store name and location (must be a Food & Home Card participating store), date of purchase, specific items bought and their costs, and the payment method.
- For utility bills: Copies of documents showing the name of the utility company, service address (must match your address on file with Devoted), payment amount, and date of payment.
- For rent and mortgage payments: Copies of documents showing the name of the landlord or bank, address of the property (must match your address on file with Devoted), monthly rent or mortgage amount, payment amount, and date of payment.
Reimbursements are limited to your monthly allowance amount, minus any funds already spent using the card that month.
