Wellbutrin SR (sustained-release bupropion) is an antidepressant that has shown promise in aiding weight loss. While not FDA-approved solely for weight loss, its active ingredient, bupropion, has been found to influence body weight, making it a potential option, particularly for individuals also requiring antidepressant treatment. This article delves into the effectiveness of Wellbutrin SR for weight loss, exploring clinical studies, mechanisms of action, potential risks, and important considerations.
Understanding Wellbutrin SR
Wellbutrin SR is a formulation of bupropion, an antidepressant that works differently from selective serotonin reuptake inhibitors (SSRIs). It functions as a norepinephrine-dopamine reuptake inhibitor (NDRI), affecting the levels of these neurotransmitters in the brain. Both Wellbutrin XL and Wellbutrin SR are approved to treat major depressive disorder. The difference between Wellbutrin XL and Wellbutrin SR lies in the dosing schedule. XL is an extended-release medication and only needs to be taken once a day, while SR is released faster.
How Bupropion Aids Weight Loss
The exact mechanism by which bupropion aids weight loss isn't fully understood, but several factors are believed to contribute:
- Appetite Suppression: Researchers believe Wellbutrin works by suppressing appetite. By targeting norepinephrine and dopamine, chemicals in the brain that affect mood and cravings, Wellbutrin may help curb emotional eating and alter the reward system related to food.
- Increased Metabolic Rate: Wellbutrin can have a stimulant effect on the central nervous system, potentially increasing metabolic rate and allowing some people to burn calories faster.
- Boost in Energy: Because it has stimulant properties, Wellbutrin can cause an uptick in energy that might lead to increased physical activity.
- Reduced Cravings: Wellbutrin is sometimes prescribed for smoking cessation because it can reduce cravings. It’s believed that the same is true for food cravings. By altering the levels of dopamine and norepinephrine in the brain, Wellbutrin can regulate reward pathways linked to cravings.
- Fat Loss: Norepinephrine increases the rate of fat loss by releasing fatty acids into the bloodstream. This reduction of fat contributes to weight loss. Dopamine works directly with appetite. There is a connection between low dopamine levels and overeating.
Clinical Evidence: Wellbutrin SR and Weight Loss
Several studies have investigated the effectiveness of bupropion SR for weight loss:
- 24-Week Placebo-Controlled Study: A 24-week multicenter, double-blind, placebo-controlled study randomized obese adults to placebo, bupropion SR 300, or 400 mg/d. Subjects were counseled on energy-restricted diets, meal replacements, and exercise. Percentage losses of initial body weight for subjects completing 24 weeks were 5.0%, 7.2%, and 10.1% for placebo, bupropion SR 300, and 400 mg/d, respectively. Compared with placebo, net weight losses were 2.2% (p = 0.0468) and 5.1% (p < 0.0001) for bupropion SR 300 and 400 mg/d, respectively. The percentages of subjects who lost >or=5% of initial body weight were 46%, 59%, and 83% (p vs. placebo < 0.0001) for placebo, bupropion SR 300, and 400 mg/d, respectively; weight losses of >or=10% were 20%, 33%, and 46% (p vs. placebo = 0.0008) for placebo, bupropion SR 300, and 400 mg/d, respectively.
- 48-Week Double-Blind, Placebo-Controlled Trial: Research indicates that bupropion SR enhances weight loss.
- Pre-Marketing Clinical Studies: In pre-marketing clinical studies of brand-name Wellbutrin SR in the treatment of major depressive disorder, mild weight loss of greater than five pounds occurred in 28% of all patients receiving the medication at either a dose of 300 mg per day or 400 mg per day. This was more than any weight loss observed in patients taking a placebo.
- Long-Term Weight Change Study: According to a clinical trial published in the Journal of Clinical Medicine, conducted by Group Health researchers, when different antidepressants were examined to understand their relationship with long-term weight change, it was found that bupropion is the only antidepressant associated with long-term weight loss; others had modest effects on weight loss. In contrast, others were associated with weight gain.
- Weight loss of more than 5%: In one study, almost 70% of participants saw weight loss of more than 5% of their body weight in 8 to 24 weeks - most of which came from lost body fat.
- Combination with Nutrition Counseling and Exercise: Another study looked at the effects of Wellbutrin weight loss when the drug was taken in combination with nutrition counseling and exercise.
These studies suggest that bupropion SR can be an effective tool for weight loss, particularly when combined with lifestyle modifications like diet and exercise.

Combination Therapy: Bupropion and Naltrexone (Contrave)
Bupropion is also available in combination with naltrexone as Contrave extended-release tablets. Contrave was FDA approved in 2014 as an adjunct to a reduced-calorie diet and increased physical activity for chronic weight management in obese or overweight adults in the presence of at least one weight-related comorbid medical condition. The dose of Contrave is a titrated dose for adults at least 18 years of age, up to a maximum of two tablets (16 mg naltrexone/180 mg bupropion) taken twice daily, and clinical studies have demonstrated more weight loss with the combination compared to bupropion alone; with one of the randomized, controlled trials contributing to its FDA approval demonstrating up to 8.2% weight loss.
The combination effect of bupropion and naltrexone on weight loss is not precise. It was proposed theoretically that naltrexone could influence the neurological pathways of the brain, while bupropion would suppress the appetite. The mechanisms through which naltrexone/bupropion combination act in reducing weight are not completely clear. α-melanocyte-stimulating hormone and β-endorphin are released by pro-opiomelanocortin producing neurons present in the hypothalamus. α-melanocyte-stimulating hormone regulates the activation of pro-opiomelanocortin and β-endorphin activates opioid receptors on pro-opiomelanocortin neurons causing autoinhibitory feedback. Bupropion monotherapy results in modest loss of weight by increasing the pro-opiomelanocortin firing. The weight loss caused by bupropion might be achieved by β-endorphin-mediated autoinhibitory feedback loop and adding naltrexone to bupropion would prevent this negative feedback loop and may help in sustained reduction of weight and achieving weight loss.
Risks and Side Effects
Like all medications, Wellbutrin SR carries potential risks and side effects:
- Common Side Effects: Tremors, vomiting, nausea, headache, dry mouth, sleep disturbances, nervousness, constipation, agitation, anxiety, confusion, delusions, psychosis, hallucinations, paranoia, rapid heart rate, dizziness, and excessive sweating.
- Serious Side Effects: Seizures, hallucinations, skin rash, high blood pressure, a fast heart rate, ocular effects, and suicidal thoughts are possible.
- Eating Disorders: Using bupropion for weight loss when an eating disorder exists can increase the risk of adverse health effects, especially seizures.
- Overdose: It is possible to overdose on bupropion, especially in cases of misuse. When someone overdoses on Wellbutrin, they may experience a seizure, which occurs in about 33% of overdoses. Other signs of a bupropion overdose include: Agitation, mental status changes, hallucinations, and cardiac arrhythmia.
- Mental Health: Labeling for Wellbutrin, like most antidepressants, was updated in 2005 to add a warning that it may cause rare cases of depression and comes with a risk of suicidal thoughts, especially in young adults.
Important Considerations
- Medical Supervision: Wellbutrin is generally safe for use under medical supervision.
- Not FDA-Approved for Weight Loss: It’s important to note that Wellbutrin is not FDA-approved as a weight loss medication, although the drug is sometimes prescribed off-label to help.
- Disordered Eating: Using bupropion to lose weight could be indicative of disordered eating, especially when other abnormal eating behaviors are present. In fact, using medications like Wellbutrin for weight control significantly increases the risk of being diagnosed with an eating disorder like anorexia nervosa (AN) or bulimia nervosa (BN).
- Withdrawal Symptoms: Stopping Wellbutrin (brand name for bupropion) suddenly may cause withdrawal symptoms. If you want to stop taking Wellbutrin, it’s important to talk to your doctor first.
- Mindful Monitoring: Monitoring weight on Wellbutrin takes a well-defined plan that combines a healthy lifestyle, nutrition, and mindfulness.
Optimizing Wellbutrin SR for Weight Loss
To get the most out of your prescription with Wellbutrin and shed those extra pounds, it is important to know some critical points:
- Timing: If taken only once a day, Wellbutrin is best taken in the morning as it can be a little stimulating, and combine with caffeine cautiously until you know how the two impact you. If taking a formulation prescribed more than once daily, the last dose should be limited to mid-afternoon, or else some insomnia may be experienced.
- Administration: Otherwise, Wellbutrin can be taken with or without food. You should swallow the medication whole; do not chew, break, or crush the tablets.
- Healthy Diet: A healthy diet is vital to maintaining a healthy weight for your body size and type. Focus on eating a balanced diet rich in fruits, vegetables, lean proteins, whole grains, and healthy fats.
- Regular Exercise: Studies show that working out regularly is great for establishing a healthy weight long-term.
- Mindfulness: Mindfulness simply means being aware of what you’re doing in the present. Paying attention to what - and how much - you’re eating can be a game changer in your weight loss journey.
- Professional Guidance: If you’re struggling with your weight or with making healthy lifestyle and diet changes, consider working with a dietician.
