Investigating Hypothalamic Leptin Gene Therapy in Rats

To investigate the efficacy of hypothalamic leptin gene therapy as an alternative method for reducing weight and its impact on bone health, a study was conducted using skeletally mature female rats. The study aimed to determine the impact of leptin-induced weight loss on bone mass, density, and microarchitecture, as well as serum biomarkers of bone turnover.

Study Design and Methods

Nine-month-old female Sprague Dawley rats were randomized into three groups: baseline control, rAAV-GFP control, and rAAV-Leptin. The baseline group was sacrificed at vector administration, while the rAAV-GFP and rAAV-Leptin groups were sacrificed 18 weeks post-vector administration.

Rats were implanted with cannulae in the 3rd ventricle of the hypothalamus and injected with either recombinant adeno-associated virus encoding the gene for rat leptin (rAAV-Leptin) or a control vector encoding green fluorescent protein (rAAV-GFP).

Throughout the study, rats were housed individually in a temperature- and light-controlled room under specific pathogen-free conditions, with ad libitum access to food and water.

Various measurements and analyses were conducted, including:

  • Body weight and food intake: Weekly measurements were taken to assess changes over time.

Results

The study found that rAAV-Leptin-treated rats lost weight (-4 ± 2%), while rAAV-GFP-treated rats gained weight (14 ± 2%) during the 18-week study. At study termination, rAAV-Leptin-treated rats weighed 17% less than rAAV-GFP-treated rats and had lower abdominal white adipose tissue weight (-80%), serum leptin (-77%), and serum IGF1 (-34%).

Cancellous bone volume fraction in distal femur metaphysis and epiphysis, and in lumbar vertebra tended to be lower in rAAV-GFP-treated rats (13.5-months-old) compared to baseline control rats (9-months-old). However, significant differences in cancellous bone or biomarkers of bone turnover were not detected between rAAV-Leptin and rAAV-GFP rats.

Statistical Analysis

Mean responses were compared between the three groups using analysis of variance (ANOVA), and two-group comparisons were made using t-tests. Longitudinal data on weekly measurements of body weight and food intake were analyzed using linear mixed models. The Benjamini and Hochberg method was used to adjust for multiple comparisons, and differences were considered significant at p ≤ 0.05.

Implications and Future Directions

The results of this study suggest that hypothalamic leptin gene therapy may be a promising alternative for reducing weight in adults, potentially mitigating the negative impact on bone health associated with traditional weight loss methods.

The Effects of Hypothalamic rAAV-Leptin Gene Therapy

rAAV-Leptin-treated rats experienced weight loss during the 18-week study, with the most significant decrease occurring in the initial four weeks post-vector administration. In contrast, control rAAV-GFP-treated rats gained weight during the study, with a greater increase in the first four weeks following vector administration.

The Impact on Bone Health

The study also examined the impact of leptin-induced weight loss on bone mass, density, and microarchitecture. While cancellous bone volume fraction tended to be lower in rAAV-GFP-treated rats compared to baseline control rats, no significant differences in cancellous bone or biomarkers of bone turnover were detected between rAAV-Leptin and rAAV-GFP rats.
