During your visits, you will see the nurse, provider (either advanced practice nurse or physician), dietitian, and exercise physiologists. We will help you set up an action plan to meet your health goals.

Program Options
OSF Weight Management Center's partnership with Health Management Resources (HMR®) allows you to choose from several non-surgical weight loss options. We are committed health care professionals able to treat even the most medically complicated patients as well as those wanting to lose weight on their own. HMR phase one classes are 13 weeks long and involve education and problem solving through group discussion. HMR phase I may be repeated if needed for more weight loss. HMR phase two will teach lifestyle skills to help achieve weight loss. Our nurse practitioner offers a strict, regimented program to ensure that you find success with medication in a healthy and suitable manner. Patients must have a BMI of 30 and higher, or a BMI of 27 with comorbidities present. OSF Little Company of Mary Medical Center offers two ways patients can reduce their obesity, boost their well-being and live a healthy lifestyle, both based on your body mass index (BMI) and other factors.

Medical Weight Loss Program
The Medical Weight Loss Program at OSF HealthCare Little Company of Mary Medical Center empowers patients with a non-surgical way to start their process of improving their well-being. An Advanced Practice Nurse (APRN) will do a thorough assessment to determine if you are a candidate for weight loss medication, and will discuss medication options with you.
Surgical Weight Loss Program
If your BMI is 35 to 39.9 and you also have co-morbidities, you qualify for weight loss surgery at OSF Medical Group.

The Weight Loss Center is located on the campus of OSF Saint Anthony Medical Center at 5666 E. State St., Rockford, Illinois.
To begin your weight loss journey, individuals are encouraged to ask their doctor for a referral to the OSF Weight Management Center. At the initial appointment, the team will gather information about the patient's medical and weight loss history, discuss program options, and create a personalized plan.
