Significant weight loss can dramatically reshape the body, often leaving the belly button looking altered. Loose skin and an uneven abdominal contour are common outcomes. Many individuals who have successfully lost weight, whether through diet and exercise or bariatric surgery, find that the appearance of their belly button is no longer aesthetically pleasing. This article explores the reasons behind these changes and discusses various solutions, ranging from lifestyle adjustments to surgical interventions.
Understanding the Pannus Stomach
A pannus stomach, also known as an "apron belly" or "mother’s apron," occurs when excess skin and fat begin to hang down from the abdomen. This condition can develop following pregnancy or significant weight changes. The excess tissue from a previous body shape can hang down, varying in length and size. While some people may feel uncomfortable with the appearance of a pannus stomach, it is possible to reduce or remove it with lifestyle changes or surgery.
Causes of Pannus Stomach
The main causes of pannus stomach include:
- Pregnancy: After delivering a baby, the extra skin necessary to accommodate the pregnancy can hang down, causing a pannus stomach.
- Obesity: Sometimes, obesity can cause fat deposits to hang down from the abdomen, resulting in a large abdominal pannus.
- Weight Loss: Rapid weight loss, such as from bariatric surgery, can leave an excess of skin that hangs from the stomach.
Managing a Pannus Stomach
A pannus stomach can be uncomfortable and, in some cases, limit a person’s ability to move. The following are some options a person can consider to help ease discomfort:
- Support Bands: These may help lift the pannus to sit higher on the body, making it easier to walk and reducing back pain.
- Keeping it Clean and Dry: A large pannus stomach can lead to ulcers and infections. Thoroughly cleaning and drying the area after getting it wet can help keep it healthy.
- Anti-Chafing Creams: Applying anti-chafing creams to the area can help prevent friction and discomfort caused by the pannus stomach rubbing against other areas of the skin.
- Lifestyle Changes: Diet and exercise can help reduce a mild case of pannus stomach.
The Impact of Pregnancy on the Belly Button
Pregnancy causes significant body changes, especially in the lower abdomen, that could be permanent. After pregnancy, every new mother wishes to get back to their old energetic self. During pregnancy, the uterus expands, stretching the navel and causing it to flatten or protrude outwards. This abdominal pressure can turn an "innie" into an "outie."

Factors Influencing Umbilical Appearance
Several factors, such as genetics, age, and weight, determine the appearance of the umbilicus. Family traits can influence the navel’s shape and size. Some people are simply predisposed to having an "innie" or "outie" belly button based on their genetic makeup. Key genetic factors influence umbilical appearance, including the umbilical hernia gene, skin and muscle development, and connective tissue strength. Moreover, the appearance of your umbilicus may vary depending on age and BMI (body mass index).
Conditions that occur during or shortly after pregnancy, such as abdominal muscle separation (diastasis recti), can also change the appearance of the umbilicus. During delivery, an umbilical hernia, which involves fat tissues poking through the abdominal wall, can also impact the belly button’s appearance.
Post-Pregnancy Considerations
After pregnancy, it is crucial to give your body enough time to recover. It is recommended to wait a minimum of twelve months before considering umbilicoplasty surgery. Similarly, it is advisable to wait twelve months after pregnancy before considering an abdominoplasty. This allows the skin to return to its pre-pregnancy state. If you develop diastasis recti (separation of abdominal muscles) during pregnancy, this duration allows the muscles to heal and regain their previous shape.
Non-Surgical Solutions for Loose Skin
Losing weight to manage overweight or obesity can significantly reduce disease risk. However, major weight loss can sometimes result in loose skin, leading to feelings of self-consciousness and discomfort.
Natural Remedies
The following natural remedies may improve skin strength and elasticity to some degree in people who have lost small to moderate amounts of weight:

- Resistance Training: Engaging in regular strength training is one of the most effective ways to build muscle mass. The Physical Activity Guidelines for Americans recommends doing strength training twice a week. While research isn’t currently available, it’s possible that an increase in muscle mass may help improve the appearance of loose skin.
- Collagen Supplementation: Collagen hydrolysate, a processed form of the collagen found in the connective tissue of animals, may have a protective effect on skin’s collagen. In a controlled 2019 study, skin elasticity, along with texture and hydration, increased significantly after 12 weeks of taking a liquid collagen supplement.
- Nutrient-Rich Diet: Certain nutrients are important for the production of collagen and other components of healthy skin:
- Protein: Adequate protein is vital for healthy skin. The amino acids proline, hydroxyproline, and glycine play a direct role in collagen production.
- Vitamin C: Vitamin C is needed for collagen synthesis and also helps protect skin from sun damage.
- Omega-3 Fatty Acids: The omega-3 fatty acids found in fatty fish and plant sources may help increase skin elasticity and have anti-aging effects.
- Hydration: Staying well-hydrated may improve the skin’s appearance. An older 2015 study found that women who increased their daily water intake had significant improvements in skin hydration and function.
Surgical Options for Belly Button Correction
When non-surgical methods are insufficient, surgical options can address the appearance of the belly button after weight loss or pregnancy. The two primary procedures are panniculectomy and umbilicoplasty, often combined with abdominoplasty.
Panniculectomy
A panniculectomy is a plastic surgery procedure designed to remove excess sagging skin on the lower abdomen. Our patients choose this procedure when they are having trouble achieving a smooth toned tummy appearance. It is essentially indicated for patients with a flat tummy with minimal excess of fat that have redundant excess of skin bellow the belly button usually secondary to pregnancies or weight loss.
Benefits of Panniculectomy
A panniculectomy in the candidate patient can:
- Tighten and remove loose skin of the lower abdomen.
- Requires minimal skin undermining, thus allowing faster recovery.
- Can be performed under local anesthesia in selected candidate patients.
- Can be a perfect finishing touch several months after childbirth.
The Panniculectomy Procedure
The procedure involves a scar that is located across the lower abdomen similar to a Tummy Tuck incision. During a panniculectomy, the belly button (umbilicus) is not changed. The separation of the rectus abdominis muscles (diastasis rectii or separation of the “six-pack muscles”) is not corrected neither during a panniculectomy. The surgeon makes an incision in the lower abdomen in the area below the “bikini line”. Dr. Long may perform liposuction of the flanks through very small skin incisions with a small delicate liposuction cannula to improve the shape of the waist. Excess skin is removed, resulting in a flatter, smoother, and trimmer stomach.
Recovery After Panniculectomy
Each patient is unique. Your recovery time will depend upon the total treatment plan. You will have some swelling and bruising, and may experience some discomfort and swelling for the first two weeks. Dr. Long uses dissolving stitches, so they do not need to be removed. You will wear an abdominal binder during the first week post surgery until most of the swelling resolves, Then you may be placed on a compression garment that is wore for 6 to 8 weeks to support your tissues as they heal. You will also have drains for several days. Most of our patients are able to return to work in 2 to 3 weeks. It will take about 4-6 weeks to return to all normal activities and exercise.

Longevity of Results
Unless you gain weight or become pregnant, you should enjoy your taut shapely figure for years to come. Dr. Long believes it is important to begin an exercise program to maintain your new look and to increase your health and fitness.
Umbilicoplasty
Umbilicoplasty is a cosmetic surgery that aims to alter your belly button’s appearance and position. The procedure involves using modern and advanced surgical techniques to correct a horizontal-shaped navel and give it a vertical shape look. Note that an umbilicoplasty only reshapes the shape of your belly button and has no effect on any other abdominal part.
Umbilicoplasty in Conjunction with Other Procedures
While a standalone umbilicoplasty is possible, it is usually included as part of an abdominoplasty. This integrates belly button repositioning with removing excess skin and fat caused by pregnancy.
Umbilicoplasty Techniques
There are basically two ways I perform umbilicus reconstruction: as a standalone procedure or incorporated into other body contouring surgeries, such as abdominoplasty.
Recovery from Umbilicoplasty
Belly button reshaping recovery time differs between patients, but it can take a day to two weeks. During this time, following the post-surgery guide I’ll provide you is crucial to minimise potential risks and complications. Expect your reconstructed navel to retain its shape after surgery.
Abdominoplasty (Tummy Tuck)
Abdominoplasty is a surgical procedure that targets skin laxity after pregnancy or significant weight loss. It involves removing excess skin and fat from the abdomen and tightening the abdominal muscles.
Belly Button Repositioning in Abdominoplasty
The first part of the abdominoplasty involves freeing the umbilicus. When loose abdominal skin is cut away to create a smooth, flat tummy, the skin is pulled taut to close the incision. As the skin is pulled down, the belly button is pulled down as well. This makes it sit too low on the tummy, which looks unnatural. Not all abdominoplasty procedures involve belly button reshaping.
Combining Procedures
Combining abdominoplasty with umbilicoplasty tightens loose skin around the abdomen and reshapes the navel. Combining c-section and umbilicoplasty requires careful consideration. While it offers the convenience of combining both childbirth and cosmetic concerns in a single surgery, it also entails increased risks, such as infection and prolonged recovery.
Alternative Medical Procedures
Although body-contouring surgery is by far the most common procedure to remove loose skin, less invasive options with a lower risk of complications include:
- Radiofrequency Treatment: Such treatments may use a combination of infrared light, radiofrequency, and massage to reduce loose skin. While this treatment doesn’t induce weight loss, it may help reduce small areas of fat cells.
- Ultrasound: A 2019 research review of ultrasound treatment for skin tightening found it to be efficacious, effective, and safe with minimal adverse effects.
Choosing the Right Surgeon and Setting Realistic Expectations
Pre-operative consultation with a board-certified plastic surgeon is absolutely critical. The goal should be to establish realistic expectations. Openly discuss your desired belly button shape and placement, allowing your surgeon to assess your suitability and discuss achievable outcomes. Addressing concerns and questions in this phase improves overall satisfaction.
